Clobetasol is a super-high-potency topical corticosteroid used to treat various inflammatory and pruritic skin conditions, including moderate to severe plaque psoriasis, eczema, atopic dermatitis, lichen planus, lichen sclerosus, cutaneous T-cell lymphoma (mycosis fungoides), and other corticosteroid-responsive dermatoses. It works by binding to the glucocorticoid receptor in skin cells, leading to decreased vasodilation and capillary permeability as well as reduced leukocyte migration. This results in potent anti-inflammatory and immunosuppressive effects. The drug also inhibits phospholipase A2 activity (reducing arachidonic acid derivatives) and suppresses pro-inflammatory gene expression while promoting anti-inflammatory genes such as interleukin 10[6][8]. It is available in multiple topical formulations (cream, ointment, gel/jelly, foam, lotion, shampoo) for application on the skin or scalp[5]. Developed decades ago as a synthetic corticosteroid derivative.
