Clocortolone pivalate is a mid-potency topical corticosteroid approved for the treatment of corticosteroid-responsive dermatoses, including eczema, dermatitis, and psoriasis. It is formulated as a 0.1% emollient cream and is suitable for both adults and pediatric patients without age restriction in the United States. The drug acts by reducing inflammation, itching, redness, and swelling associated with various skin conditions through its anti-inflammatory and immunosuppressive effects. The precise mechanism involves induction of phospholipase A2 inhibitory proteins (lipocortins), which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes[5][6][3]. Clocortolone pivalate was developed to achieve mid-range potency with good skin penetration while minimizing irritation; it has a unique structure among corticosteroids due to specific halogenation patterns[5][6]. It was approved by the FDA in 1977[7].
