Overview

Clodronic acid is a first-generation, non-nitrogen-containing bisphosphonate used primarily for the management of hypercalcemia and osteolysis associated with malignant tumors, such as breast cancer and multiple myeloma. Developed and marketed by companies including Bioprojet (under the brand name Clastoban), it acts by inhibiting bone resorption. Unlike nitrogen-containing bisphosphonates (e.g., zoledronic acid), clodronic acid is metabolized within osteoclasts into a non-hydrolyzable analog of adenosine triphosphate (ATP), specifically adenosine 5'-(beta,gamma-dichloromethylene)triphosphate (AppCp). This metabolite accumulates and inhibits mitochondrial ATP/ADP translocase, leading to osteoclast apoptosis and a subsequent reduction in bone turnover. It is available in both oral and intravenous formulations.
