Overview

This is a **combination regimen** consisting of clomiphene citrate (a selective estrogen receptor modulator), gonadotropins (exogenous hormones including follicle-stimulating hormone and/or luteinizing hormone), and metformin (an insulin-sensitizing agent). It is employed in the management of **infertility in women with polycystic ovary syndrome (PCOS)**, particularly those who are resistant to clomiphene citrate alone. - **Clomiphene citrate** acts primarily as an estrogen receptor antagonist in the hypothalamus, increasing the secretion of gonadotropin-releasing hormone (GnRH) and consequently stimulating pituitary release of FSH and LH, thereby promoting follicular development and ovulation. - **Gonadotropins** (FSH and/or LH, administered as injectable medications) provide direct stimulation of the ovaries, supporting follicular growth and ovulation in women unresponsive to oral ovulation inducers. - **Metformin** improves insulin sensitivity, lowers circulating insulin levels, and can restore ovulation in women with PCOS by counteracting the hyperinsulinemic state often present in PCOS, thereby reducing androgen production and improving ovarian function. The combination is most often used for women with PCOS who have not responded to clomiphene monotherapy (so-called clomiphene-resistant PCOS). Studies show that while clomiphene citrate + metformin improves ovulation and pregnancy rates compared to clomiphene alone[1][2][3], the addition of injectable gonadotropins results in even higher ovulation and pregnancy rates, but with increased risks of multiple pregnancy and ovarian hyperstimulation[5][6].
