Overview

This is a combination of four anticonvulsant and sedative drugs—clonazepam, levetiracetam, midazolam, and phenobarbital. All four are used in the management of seizures, particularly in refractory or status epilepticus cases where monotherapy is insufficient. - **Clonazepam** is a benzodiazepine that enhances GABAergic inhibition by binding to the benzodiazepine site on the GABA-A receptor. - **Levetiracetam** is an antiepileptic drug that binds to synaptic vesicle protein SV2A, modulating neurotransmitter release. - **Midazolam** is a short-acting benzodiazepine acting as a positive allosteric modulator at the GABA-A receptor. - **Phenobarbital** is a barbiturate that increases synaptic inhibition via prolonged opening of chloride channels at the GABA-A receptor and also inhibits glutamate-induced depolarizations. This combination may be used in severe or refractory seizure conditions such as status epilepticus when multiple mechanisms are targeted for maximal seizure control.
