Clopidogrel is a thienopyridine-class antiplatelet medication used to reduce the risk of heart disease and stroke in those at high risk. It is a prodrug that requires metabolic activation by the cytochrome P450 system, specifically the CYP2C19 enzyme, to produce its active thiol metabolite. This active metabolite irreversibly inhibits the P2Y12 subtype of adenosine diphosphate (ADP) receptors on platelet membranes. By blocking these receptors, clopidogrel prevents the activation of the glycoprotein IIb/IIIa receptor complex, thereby inhibiting platelet aggregation for the remainder of the platelet's lifespan. It is a cornerstone of dual antiplatelet therapy (DAPT), typically administered alongside aspirin, for patients undergoing percutaneous coronary intervention (PCI) or those with acute coronary syndrome (ACS).
