clopidogrel (Farmak)

Clopidogrel bisulfate, manufactured by Joint Stock Company (JSC) Farmak, is a generic small molecule antiplatelet agent. It is a prodrug that requires hepatic metabolism, primarily via the CYP2C19 enzyme, to form its active thiol metabolite. This active metabolite specifically and irreversibly binds to the P2Y12 subtype of adenosine diphosphate (ADP) receptors on the platelet surface. This binding prevents ADP from activating the glycoprotein IIb/IIIa receptor complex, which is the final common pathway for platelet aggregation. By inhibiting platelet aggregation, clopidogrel reduces the risk of atherothrombotic events, such as myocardial infarction and stroke, in high-risk patients with established atherosclerosis. JSC Farmak's formulation is a 75 mg film-coated tablet developed as a generic equivalent to the reference brand Plavix.
