Overview
Clopidogrel + aspirin is a dual antiplatelet therapy (DAPT) regimen evaluated in the TEG-CABG trial at Rigshospitalet, Denmark. Clopidogrel is a thienopyridine small molecule that acts as a prodrug; its active metabolite irreversibly inhibits the P2Y12 adenosine diphosphate (ADP) receptor on platelets. Aspirin (acetylsalicylic acid) irreversibly inhibits cyclooxygenase-1 (COX-1), thereby preventing the synthesis of thromboxane A2. The TEG-CABG trial (2008-2013) was a Phase 3 study investigating whether this combination, compared to aspirin monotherapy, could improve saphenous vein graft patency and reduce thromboembolic complications in patients undergoing coronary artery bypass graft (CABG) surgery who were preoperatively identified as hypercoagulable via thrombelastography (maximum amplitude > 69 mm).
