Clopidogrel + lansoprazole is a combination of two pharmaceutical agents: clopidogrel, an antiplatelet drug, and lansoprazole, a proton pump inhibitor (PPI). Clopidogrel is a prodrug that requires activation by the hepatic enzyme CYP2C19 to form its active metabolite. This metabolite irreversibly inhibits the P2Y12 ADP receptor on platelets, thereby preventing platelet aggregation and reducing the risk of thrombotic events such as myocardial infarction and stroke[6]. Lansoprazole acts by inhibiting the H+/K+ ATPase enzyme in gastric parietal cells, leading to reduced gastric acid secretion. The combination may be used in patients requiring both antiplatelet therapy (to prevent cardiovascular events) and gastroprotection (to reduce gastrointestinal bleeding risk). However, there is evidence that some PPIs can interfere with clopidogrel’s activation via CYP2C19 inhibition; while omeprazole and esomeprazole are most implicated in this interaction, lansoprazole has less effect but still carries some risk[1][5][8].
