Overview

Otofar is a multi-ingredient otic solution developed and marketed by Laboratorios Mallén for the treatment of otitis externa and otitis media complicated by bacterial or fungal infections, pruritus, or pain. It is a fixed-dose combination containing four active ingredients: clotrimazole (an imidazole antifungal), neomycin (an aminoglycoside antibiotic), polymyxin B (a polypeptide antibiotic), and tetracaine (an ester-type local anesthetic). Clotrimazole inhibits fungal lanosterol 14-alpha demethylase to disrupt cell membrane synthesis, while neomycin and polymyxin B target bacterial protein synthesis and outer membrane integrity, respectively. Tetracaine blocks voltage-gated sodium channels to provide rapid local pain relief.
