Cloxacillin + fosfomycin is a combination of two antibiotics used primarily for the treatment of methicillin-sensitived Staphylococcus aureus (MSSA) bacteremia. Cloxacillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. Fosfomycin is a broad-spectrum antibiotic that exerts its bactericidal effect by inhibiting the enzyme UDP-N-acetylglucosamine enolpyruvyl transferase (MurA), which catalyzes an early step in bacterial cell wall biosynthesis. The combination has been studied in clinical trials for potential synergistic effects, aiming to improve outcomes in severe MSSA infections[1][4][5]. However, recent phase III-IV clinical trial data indicate that cloxacillin plus fosfomycin did not achieve significantly better treatment success at day 7 compared to cloxacillin monotherapy for MSSA bacteremia[1][4].
