CMD63 is an autologous chimeric antigen receptor (CAR) T-cell therapy being developed by Chulalongkorn University for the treatment of relapsed or refractory B-cell acute lymphoblastic leukemia (B-ALL). The therapy involves the genetic modification of a patient's own T cells using a lentiviral vector to express a CAR that targets the B-lymphocyte antigen CD19. A defining feature of CMD63 is the incorporation of an interleukin-7 receptor alpha (IL-7Rα) signaling domain within the chimeric construct. This design is intended to provide cell-intrinsic cytokine support, mimicking the survival and homeostatic signals normally provided by IL-7. By activating the IL-7Rα pathway, the therapy aims to enhance the expansion, metabolic fitness, and long-term persistence of the CAR-T cells, potentially overcoming the common clinical challenge of T-cell exhaustion and providing more durable remissions in pediatric patients.
