CMS121 is a small molecule drug candidate derived from the flavonol fisetin, developed as a neuroprotective agent for the treatment of Alzheimer's disease and age-related dementia. It acts primarily by partially inhibiting fatty acid synthase (FASN), leading to reduced lipid peroxidation and neuroinflammation—processes implicated in cognitive decline and neurodegeneration. In preclinical models, CMS121 has demonstrated efficacy in maintaining cognitive function, reducing metabolic and transcriptional markers of aging in the brain, and normalizing mitochondrial acetyl-CoA metabolism. Beyond its geroneuroprotective effects, CMS121 also shows promise for improving metabolic health parameters such as glucose metabolism, lipid profiles, liver inflammation, and renal protection in animal models of obesity and diabetes. The compound has received FDA Investigational New Drug (IND) approval and completed phase 1 clinical trials assessing safety and tolerability in healthy adults[1][3][5][7].
