CMV pp65-LAMP mRNA-pulsed autologous DCs + GM-CSF is an investigational autologous cellular immunotherapy and cancer vaccine. It is manufactured by deriving a patient's own dendritic cells (DCs) from peripheral blood mononuclear cells (PBMCs) and electroporating them with mRNA encoding the human cytomegalovirus (CMV) matrix protein pp65 fused to the lysosomal-associated membrane protein (LAMP). This fusion protein, often referred to as pp65-flLAMP (full-length) or pp65-shLAMP (short), is designed to direct the antigen to the lysosomal compartment for enhanced MHC class II presentation, thereby stimulating a robust response from both CD4+ and CD8+ T cells. The antigen-loaded DCs are admixed with Granulocyte-Macrophage Colony-Stimulating Factor (GM-CSF) as an adjuvant to promote DC maturation and recruitment at the injection site. The therapy specifically targets the CMV pp65 antigen, which is frequently expressed in glioblastoma multiforme (GBM) tumor cells but absent in normal brain tissue. It is currently being evaluated in the Phase II I-ATTAC trial for patients with newly diagnosed, CMV-positive, MGMT-unmethylated glioblastoma.
