The **CMV pp65 peptide vaccine** is an investigational, HLA-A*02-restricted synthetic peptide vaccine developed for prevention and control of cytomegalovirus infection or reactivation after allogeneic hematopoietic stem cell transplantation. Its active antigen is the CMV phosphoprotein 65 epitope **NLVPMVATV** corresponding to pp65 residues 495–503. The peptide was administered as a 300 µg subcutaneous water-in-oil emulsion with incomplete Freund's adjuvant Montanide ISA-51, with granulocyte-macrophage colony-stimulating factor co-administered as an immune adjuvant. Vaccination is intended to expand CMV pp65-specific cytotoxic CD8-positive T-cell responses and support CMV immune reconstitution in high-risk transplant recipients, including patients receiving grafts from CMV-seronegative donors. In the UL-CMV-1 Phase I study, four biweekly vaccinations were generally well tolerated and most evaluable patients with CMV antigenemia cleared antigenemia.
