Overview

CNOP is a multi-agent chemotherapy regimen primarily used for the treatment of non-Hodgkin lymphoma (NHL). It is a combination of four drugs: cyclophosphamide (an alkylating agent), mitoxantrone (an anthracenedione antineoplastic), vincristine (a vinca alkaloid), and prednisone (a corticosteroid). The regimen was developed as an alternative to the standard CHOP therapy, substituting doxorubicin with mitoxantrone to reduce potential cardiotoxicity and side effects such as alopecia and gastrointestinal toxicity. Mechanistically, the regimen targets multiple cellular processes: cyclophosphamide cross-links DNA, mitoxantrone intercalates into DNA and inhibits topoisomerase II, vincristine disrupts microtubule assembly during mitosis, and prednisone induces apoptosis in malignant lymphoid cells via glucocorticoid receptor activation. Although CNOP is generally better tolerated than CHOP, clinical trials and meta-analyses have shown it to be less effective in terms of complete response rates and overall survival for aggressive lymphomas.
