CNP-PLP178-191 is a tolerogenic immune-modifying nanoparticle (TIMP) developed by Cour Pharmaceutical Development Company. It consists of biodegradable poly(d,l-lactic-co-glycolic acid) (PLGA) nanoparticles, typically 400-800 nm in diameter, that encapsulate the myelin proteolipid protein (PLP) peptide fragment 178-191. The drug is designed to induce antigen-specific immune tolerance for the treatment of multiple sclerosis (MS). Upon intravenous administration, the nanoparticles are selectively taken up by MARCO-expressing myeloid cells in the spleen and liver. These cells subsequently undergo apoptosis, leading to the activation of the cGAS/STING pathway and the local secretion of type I interferons. This process promotes the expansion of antigen-specific FoxP3+ regulatory T cells (Tregs) and Tr1 cells, which suppress autoreactive T cell responses against myelin without causing systemic immunosuppression. Preclinical studies have demonstrated its efficacy in preventing and treating disease in the experimental autoimmune encephalomyelitis (EAE) model.
