Cobicistat + midazolam is a combination of two small molecule drugs used together for pharmacokinetic boosting. Cobicistat is a potent inhibitor of cytochrome P450 3A (CYP3A) enzymes, primarily used to increase the plasma concentrations and prolong the effects of drugs metabolized by CYP3A, such as certain antiretrovirals and benzodiazepines. Midazolam is a short-acting benzodiazepine that acts as an agonist at the gamma-aminobutyric acid type A (GABA-A) receptor, producing sedative, anxiolytic, anticonvulsant, and muscle relaxant effects. When administered together, cobicistat inhibits the metabolism of midazolam via CYP3A4 inhibition, resulting in increased exposure and prolonged half-life of midazolam. This combination has been explored in specific clinical scenarios such as drug-resistant epilepsy requiring stable oral midazolam concentrations after intravenous therapy[1][5]. However, coadministration—especially with oral midazolam—is generally contraindicated due to risk for serious or life-threatening sedation or respiratory depression unless under close medical supervision[2][6].
