Cobratoxin is a 71-amino acid long-chain post-synaptic neurotoxin isolated from the venom of the Thailand cobra (*Naja kaouthia*). It functions as a high-affinity, reversible antagonist of nicotinic acetylcholine receptors (nAChRs), specifically targeting the alpha-7 (α7) and muscle-type (α1β1δε) subtypes. While naturally a potent paralytic agent, chemically modified or detoxified versions of the peptide, such as **RPI-78M**, are being developed for therapeutic applications. These modified forms retain neuromodulatory, anti-inflammatory, and analgesic properties without the systemic toxicity of the raw venom. Clinical development has primarily focused on neurodegenerative and autoimmune conditions, including **adrenomyeloneuropathy (AMN)** and **multiple sclerosis (MS)**, where the drug is thought to modulate immune responses and provide neuroprotection. It has also been investigated for potential use in chronic kidney disease and as an antiviral agent.
