Cockroach sublingual immunotherapy (SLIT) is an investigational form of allergen-specific immunotherapy designed to treat allergic rhinitis and asthma caused by sensitization to cockroach allergens. The therapy involves administering gradually increasing doses of purified or standardized cockroach allergen extracts under the tongue (sublingually), with the goal of inducing immune tolerance and reducing allergic symptoms over time. Cockroach SLIT is not currently FDA-approved in the United States; only SLIT tablets for grass pollen, ragweed pollen, and dust mites are approved. However, research and clinical trials have explored its safety and efficacy as an alternative to traditional allergy shots (subcutaneous immunotherapy). The mechanism of action involves modulation of the immune response—specifically increasing regulatory T-cell activity and shifting antibody production from IgE toward IgG4—thereby reducing hypersensitivity reactions upon exposure to environmental cockroach allergens[1][3][5]. Primary indications under investigation include allergic rhinitis and asthma due to sensitization to German or American cockroaches.
