Overview

Codonopsis pilosula is a perennial herb native to East Asia and widely used in Traditional Chinese Medicine (TCM) as an adaptogen and tonic. The root is the primary medicinal part and is traditionally employed to replenish vital energy (Qi), strengthen the spleen and lungs, nourish blood and fluids (hematopoiesis), support digestive health (loss of appetite/diarrhea), improve respiratory function (cough/asthma/chronic bronchitis), enhance immune function and modulate inflammation[3][4][5][7]. It is sometimes called "poor man's ginseng" due to its similar but milder effects compared to Panax ginseng; however it does not contain ginsenosides[7]. Modern research indicates that C. pilosula contains polysaccharides (notably immunomodulatory ones), saponins and flavonoids as active constituents. Its mechanisms include immunomodulation (increasing IgG/IgM/IFN-γ/IL-2/IL-4 levels; enhancing macrophage activity; shifting TH2→TH1 response); anti-inflammatory effects; antioxidant activity; regulation of apoptosis; protection against oxidative stress; improvement of cardiovascular health by reducing endothelial dysfunction/inflammation/lipid peroxidation; neuroprotection in animal models with some evidence for cognitive enhancement in humans when combined with other herbs[3][5][7]. It has also shown potential benefits for cancer patients undergoing chemotherapy/radiotherapy by supporting immunity. In TCM/Korean medicine it is used for chronic fatigue syndrome-like symptoms as well as gynecological disorders. Clinical evidence remains limited outside traditional use or combination formulas.
