**Colchicine + theophylline** is a combination of two small molecule drugs. Colchicine is an alkaloid used in the treatment of conditions such as gout and familial Mediterranean fever; it acts primarily by inhibiting tubulin polymerization and modulating microtubule dynamics, which suppresses neutrophil activity and inflammation. Theophylline is a methylxanthine bronchodilator used in respiratory diseases like asthma and COPD; it acts as a nonselective phosphodiesterase inhibitor, increasing intracellular cyclic AMP, and as an adenosine receptor antagonist[2][3][6][7]. When co-administered, colchicine can suppress hepatic CYP1A2, for which theophylline is a sensitive probe substrate[1]. Clinical studies have examined the pharmacokinetic interactions, showing that steady-state colchicine does not meaningfully affect theophylline concentrations in healthy adults[2][4][7]. Synergistic immunomodulatory effects have been observed – in vitro colchicine together with oral theophylline corrects suppressor cell function abnormalities in asthmatic patients, raising potential utility in extrinsic asthma[3]. This combination is used experimentally or investigationally, primarily in clinical pharmacology research or potential off-label immunomodulatory indications.
