Colchimax is a fixed-dose combination drug containing colchicine and tiemonium methylsulfate, primarily used in France for the treatment of gout and related inflammatory conditions. In the context of the COLIN (Colchicine in Acute Myocardial Infarction) trial conducted by the University Hospital, Montpellier, Colchimax was evaluated for its potential to attenuate the inflammatory response and improve cardiac remodeling in patients presenting with ST-segment elevation myocardial infarction (STEMI). The colchicine component acts by inhibiting tubulin polymerization and the NLRP3 inflammasome, thereby reducing the production of pro-inflammatory cytokines such as IL-1β. Tiemonium methylsulfate is included as an antispasmodic to improve gastrointestinal tolerance, a common limiting factor for colchicine therapy. The Phase 2 COLIN trial demonstrated that colchicine could reduce C-reactive protein (CRP) levels in the acute phase of MI, although it did not significantly impact infarct size as measured by MRI in that specific study.
