**Colesevelam + pioglitazone** is a combination regimen used in the management of **type 2 diabetes mellitus**. **Colesevelam** is a bile acid sequestrant that lowers LDL cholesterol and improves glycemic control by binding bile acids in the intestine, thereby reducing their reabsorption. **Pioglitazone** is a thiazolidinedione that enhances insulin sensitivity through activation of peroxisome proliferator-activated receptor gamma (PPARγ), which regulates glucose and lipid metabolism. This combination improves glycemic parameters (HbA1c, fasting plasma glucose) and lipid profiles (reducing LDL cholesterol, total cholesterol, apolipoprotein B, non-HDL cholesterol and raising HDL cholesterol), and is generally well tolerated with mild or moderate adverse events[1][2]. The main indication is for **adults with type 2 diabetes** who have inadequate glycemic control on pioglitazone-based therapy.
