Colesevelam + simvastatin is a combination therapy used to lower elevated cholesterol levels, particularly low-density lipoprotein cholesterol (LDL-C), in patients with hypercholesterolemia. Colesevelam is a bile acid sequestrant that binds bile acids in the intestine, preventing their reabsorption and promoting increased clearance of LDL-C from the blood. Simvastatin is an HMG-CoA reductase inhibitor (statin) that blocks the rate-limiting step in cholesterol biosynthesis, leading to reduced hepatic cholesterol synthesis and increased uptake of LDL by upregulating hepatic LDL receptors. The combination provides additive reductions in LDL-C compared to either agent alone and is well tolerated.
