Colestilan is a non-absorbed, anion-exchange resin developed by Mitsubishi Tanabe Pharma Corporation for the treatment of hyperphosphatemia in patients with chronic kidney disease (CKD). Chemically, it is a cross-linked copolymer of 2-methylimidazole and epichlorohydrin. It functions as a phosphate binder within the gastrointestinal tract, where it binds to dietary phosphate, bile acid anions, and urate, preventing their absorption into the systemic circulation. This mechanism effectively lowers serum phosphate levels in patients with CKD, particularly those on dialysis. While it received marketing authorization in the European Union under the brand name BindRen in 2013, the authorization was later withdrawn for commercial reasons. It remains an important therapeutic option in other regions, such as Japan, and has been investigated for use in pediatric populations with CKD stages 3b-5.
