Colistin + cefoperazone-sulbactam is an investigational antibiotic combination regimen being evaluated for the treatment of multidrug-resistant (MDR) Acinetobacter baumannii ventilator-associated pneumonia (VAP). The regimen combines colistin, a polymyxin antibiotic that disrupts the bacterial outer membrane by binding to lipopolysaccharides, with cefoperazone-sulbactam. Cefoperazone is a third-generation cephalosporin that inhibits bacterial cell wall synthesis by targeting penicillin-binding proteins (PBPs), while sulbactam is a beta-lactamase inhibitor that prevents the enzymatic degradation of cefoperazone and exhibits direct bacteriostatic activity against Acinetobacter species. This combination therapy aims to improve clinical outcomes and survival rates in critically ill patients with severe nosocomial infections where monotherapy may be insufficient.
