This is a combination of three antimicrobial agents—colistin, tobramycin, and nystatin. Colistin (also known as polymyxin E) is a polymyxin antibiotic that disrupts the bacterial cell membrane of Gram-negative bacteria, leading to cell death. Tobramycin is an aminoglycoside antibiotic that inhibits protein synthesis by binding to the 30S ribosomal subunit in susceptible bacteria, particularly effective against Pseudomonas aeruginosa and other Gram-negative organisms. Nystatin is a polyene antifungal agent that binds to ergosterol in fungal cell membranes, increasing permeability and causing leakage of cellular contents; it is primarily active against Candida species. This combination may be used for topical or inhalational therapy in settings where mixed infections with resistant Gram-negative bacteria (such as Pseudomonas aeruginosa) and fungi are suspected or confirmed—for example, in cystic fibrosis patients with chronic airway colonization/infection[1][3][4]. The use of colistin-tobramycin combinations has been shown to be more effective than monotherapy for eradicating biofilms formed by Pseudomonas aeruginosa[1][2]. Nystatin adds antifungal coverage.
