COM503 is a fully human, high-affinity anti-IL-18 binding protein (IL-18BP) monoclonal antibody developed as a potential first-in-class immuno-oncology therapy. Its mechanism of action involves blocking the endogenous IL-18 binding protein, which normally neutralizes the proinflammatory cytokine IL-18 in the tumor microenvironment. By inhibiting IL-18BP, COM503 releases and restores natural IL-18 activity locally within tumors, thereby enhancing T-cell and NK-cell activation and potentiating anti-tumor immune responses. This approach aims to overcome limitations associated with systemic cytokine therapies by focusing immune modulation within the tumor microenvironment while minimizing systemic toxicity. COM503 is being evaluated in Phase 1 clinical trials as monotherapy and in combination with zimberelimab (an anti-PD1 antibody) for patients with advanced or metastatic solid tumors[1][3][6][7][8].
