COMBATAC protocol

The COMBATAC (COMBined Anticancer Treatment of Advanced Colorectal cancer) protocol is a multimodal therapeutic strategy developed by the University of Regensburg for patients with K-ras wild-type colorectal or appendiceal peritoneal carcinomatosis. The regimen integrates perioperative systemic targeted therapy with surgical intervention. It consists of preoperative systemic chemotherapy (FOLFOX or FOLFIRI) combined with the EGFR-targeting monoclonal antibody cetuximab, followed by cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) using a bidirectional perfusion of oxaliplatin, 5-fluorouracil, and folinic acid. The treatment concludes with postoperative systemic chemotherapy and cetuximab. This approach aims to address both systemic micrometastases and localized peritoneal disease by combining systemic control with aggressive local treatment.
