Combined HLA-matched bone marrow and kidney transplantation is a specialized therapeutic protocol developed by Massachusetts General Hospital for patients with plasma cell dyscrasias, such as multiple myeloma or systemic AL amyloidosis, who also suffer from end-stage renal disease (ESRD). The regimen aims to achieve two primary goals: the eradication of the underlying hematologic malignancy through a graft-versus-tumor effect and the restoration of renal function through transplantation. A key feature of this approach is the induction of immune tolerance, which potentially allows the recipient to accept the donor kidney without the need for lifelong immunosuppressive therapy. The protocol involves a conditioning phase using low-dose total body irradiation (TBI), equine anti-thymocyte globulin (ATG), and tacrolimus, followed by the simultaneous or near-simultaneous infusion of donor bone marrow and surgical implantation of the donor kidney from the same HLA-matched related donor.
