The Complete Motor Sparing Protocol (CMP) is a regional anesthesia regimen developed by Loyola University specifically for managing postoperative pain in patients undergoing total hip arthroplasty (THA). The protocol aims to provide effective pain relief while minimizing the motor nerve blockade often associated with traditional techniques like the Fascia Iliaca block. CMP involves three distinct ultrasound-guided nerve blocks: the cluneal nerve block, the pericapsular nerve group (PENG) block, and the lateral femoral cutaneous nerve (LFCN) block. Each component of the protocol utilizes 0.25% bupivacaine, a long-acting local anesthetic. Bupivacaine works by binding to the intracellular portion of voltage-gated sodium channels in neuronal membranes, which inhibits sodium ion influx and prevents the initiation and conduction of nerve impulses. By targeting sensory nerves that innervate the hip joint and surrounding skin without affecting major motor nerves, CMP facilitates earlier patient mobilization and potentially reduces the need for systemic opioids.
