Compound 21 + losartan is an experimental combination therapy consisting of two small molecules targeting the renin-angiotensin system. Compound 21 (C21) is a selective angiotensin II type 2 receptor (AT2R) agonist, while losartan is an angiotensin II type 1 receptor (AT1R) antagonist (ARB). The combination has been studied in preclinical models for its potential additive or synergistic effects in reducing albuminuria and renal fibrosis in diabetic nephropathy and improving vascular function and reducing cardiac hypertrophy in hypertension. Mechanistically, C21 activates AT2 receptors to counteract many of the deleterious effects mediated by AT1 activation, such as inflammation and fibrosis. Losartan blocks AT1 receptors to reduce blood pressure and organ damage. Together, they may provide enhanced protection against kidney injury and cardiovascular remodeling by modulating different arms of the renin-angiotensin system[1][2][6].
