Overview

Compound 386 granule is a pharmaceutical formulation containing 3,4-O-dicaffeoylquinic acid, a compound isolated from the plant Laggera alata. It is being investigated for its antiviral and hepatoprotective effects in the treatment of Hepatitis B Virus-related Decompensated Cirrhosis. Its mechanism of action involves inhibiting the production of Hepatitis B surface antigen (HBsAg) and Hepatitis B e-antigen (HBeAg), significantly decreasing the content of Hepatitis B virus covalently closed circular DNA (HBV cccDNA), and upregulating heme oxygenase-1 (HO-1). The upregulation of HO-1 is believed to contribute to its anti-HBV effect by reducing the stability of the HBV core protein, thereby blocking the refill of nuclear HBV cccDNA. Additionally, its hepatoprotective properties may stem from antioxidative and anti-inflammatory activities, also mediated by HO-1 induction.
