Compound alpha-ketoacid tablets are a nutritional therapeutic used primarily in patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD) to manage protein metabolism. The tablets contain a combination of calcium salts of keto-analogues of essential amino acids (isoleucine, leucine, phenylalanine, valine, and methionine) along with specific essential amino acids (lysine, threonine, tryptophan, histidine, and tyrosine). The mechanism of action involves the transamination of these keto-analogues into their corresponding essential amino acids by utilizing nitrogen from non-essential amino acids, such as urea. This process effectively recycles nitrogenous waste, reduces urea production, and helps maintain nitrogen balance while allowing for a low-protein diet, which can delay the progression of renal failure and mitigate uremic symptoms. In the context of peritoneal dialysis, they are evaluated for their ability to improve nutritional status and combat malnutrition.
