Compound Shengmai Chenggu Capsule is a Traditional Chinese Medicine (TCM) hospital preparation developed by the First Affiliated Hospital of Guangzhou University of Chinese Medicine. It is specifically indicated for the treatment of osteonecrosis of the femoral head (ONFH). The formulation is based on the classic Shengmai formula, which includes *Panax ginseng*, *Ophiopogon japonicus*, and *Schisandra chinensis*, and is further enriched with bone-strengthening and blood-activating herbs such as *Drynaria fortunei* and *Epipedium brevicornu*. The drug works by improving local blood microcirculation, promoting the expression of osteogenic factors like bone morphogenetic protein 2 (BMP-2) and vascular endothelial growth factor (VEGF), and inhibiting osteoclast activity via the OPG/RANKL pathway. It is used clinically to prevent femoral head collapse and promote bone tissue repair in patients with early-to-middle stage necrosis.
