Concentrated bone marrow aspirate (BMAC) is an autologous regenerative cell therapy developed by the Mayo Clinic for the treatment of symptomatic knee osteoarthritis. The therapy involves harvesting approximately 52 milliliters of bone marrow from the patient's iliac crest, which is then processed via automated centrifugation to concentrate mesenchymal stem cells (MSCs), hematopoietic stem cells, and various bioactive growth factors. This concentrate is typically combined with platelet-poor plasma and administered via intra-articular injection into the arthritic knee. The mechanism of action involves the modulation of the intra-articular inflammatory environment, promotion of cartilage regeneration, and enhancement of subchondral bone remodeling through paracrine signaling and cellular differentiation. Mayo Clinic has evaluated this therapy in randomized controlled trials comparing its efficacy against saline placebos and conventional platelet-rich plasma (PRP).
