Concentrated secretome refers to a cell-free therapeutic product derived by concentrating the totality of bioactive molecules released by cultured cells—most commonly mesenchymal stem/stromal cells (MSCs) or specifically adipose stromal cells (ASCs)—into the extracellular space. This includes proteins, cytokines, growth factors, microvesicles, and exosomes. The preparation is designed to harness the paracrine effects of stem/stromal cells without administering live cells themselves. Concentrated ASC-derived secretome has demonstrated preclinical efficacy in reducing lung injury in models of influenza A infection by limiting inflammation and tissue damage; its mechanism involves modulation of inflammatory mediators such as angiopoietin 2 and programmed death ligand 1 (PDL1), though suppression of PDL1 is not solely responsible for its therapeutic effect[1][2][3]. The product can be engineered or "primed" with inflammatory stimuli to enhance specific anti-inflammatory or regenerative properties[1][3]. Concentrated secretomes are being explored as alternatives to direct cell therapy for a range of conditions including acute lung injury, chronic inflammatory diseases, neurological disorders, arthritis, spinal cord injuries, and more[2][3].
