This combination therapy was evaluated in a clinical trial (NCT00806910) sponsored by the Albert Einstein Healthcare Network for the treatment of acute decompensated heart failure (ADHF) and hyponatremia. The regimen consists of **conivaptan**, a dual vasopressin V1A and V2 receptor antagonist; **furosemide**, a loop diuretic; and **nesiritide**, a recombinant human B-type natriuretic peptide (BNP). Conivaptan promotes aquaresis by blocking the V2 receptors in the renal collecting ducts, while furosemide induces natriuresis by inhibiting the Na-K-2Cl symporter in the loop of Henle. Nesiritide acts as a vasodilator and further promotes natriuresis by binding to natriuretic peptide receptor-A. The study aimed to determine if the addition of conivaptan to standard-of-care diuretics and vasodilators would enhance diuresis and improve renal function in patients with significant fluid overload and low sodium levels.
