Continuous adductor canal block (CACB) is a regional anesthesia procedure being studied by researchers at the University of Toronto and Mount Sinai Hospital for the management of postoperative pain in patients undergoing outpatient primary total knee arthroplasty (TKA). The intervention involves the continuous infusion of 0.2% ropivacaine, a long-acting small molecule local anesthetic, through an indwelling perineural catheter placed in the adductor canal. Ropivacaine works by blocking voltage-gated sodium channels in nerve fibers, which prevents the transmission of pain signals. This technique aims to provide prolonged analgesia beyond the 24-hour limit of single-shot blocks, thereby reducing opioid consumption and improving the quality of recovery in the first 48-72 hours post-surgery.
