The combination drug **copanlisib + rituximab + bendamustine** is a multi-agent regimen investigated primarily for the treatment of relapsed/refractory diffuse large B-cell lymphoma (DLBCL) and chronic lymphocytic leukemia (CLL) in patients ineligible for transplant or CAR-T therapy[1][2][3][5][7][10]. - **Copanlisib** is a small molecule intravenous inhibitor of class I phosphoinositide 3-kinases (PI3K), targeting isoforms PI3K-alpha, -beta, -gamma, and -delta, thereby blocking key signaling pathways involved in B-cell proliferation and survival, apoptosis, and cell cycle progression[4]. - **Rituximab** is a monoclonal antibody targeting CD20 on B cells, leading to cell lysis through complement-dependent cytotoxicity, antibody-dependent cellular cytotoxicity, and induction of apoptosis. - **Bendamustine** is an alkylating agent with antimetabolite activity, causing DNA crosslinking and strand breaks, leading to cell death, and has unique mechanisms of activating apoptotic and non-apoptotic death pathways[6]. This regimen (often called copa-BR or copa-RB) has been studied in multicenter phase II trials. It showed limited efficacy in transplant-ineligible relapsed/refractory DLBCL, with poor tolerability and significant adverse effects (notably neutropenia, infection, thrombocytopenia), resulting in premature termination of clinical studies in this setting[1][3][7][9][10]. It has also been evaluated (PHASE2) in chronic lymphocytic leukemia and small lymphocytic lymphoma[2][5][8].
