COR-1167 is a first-in-class, long-acting peptide agonist of the corticotropin-releasing hormone receptor 2 (CRF2). It was developed as a stable analog of urocortin-2, a natural CRF family peptide with cardiovascular protective effects. Unlike native urocortin-2, which has a short half-life and requires intravenous administration, COR-1167 is designed for once-daily subcutaneous injection using an autoinjector. The drug aims to decrease left atrial pressure and central venous pressure while improving cardiac output in patients with heart failure—particularly during the high-risk period following an episode of worsening heart failure (WHF). Preclinical studies in rat and sheep models have shown that chronic CRF2 activation by COR-1167 improves both cardiac and renal function on top of standard therapies such as furosemide or empagliflozin. Clinical proof-of-concept for the mechanism has been demonstrated with intravenous urocortin-2 in heart failure patients. Phase 1 clinical trials have been completed successfully; phase 2 trials are planned for adjunctive treatment in heart failure[1][3][5][6][8].
