Overview

COR588 is a second-generation, selective, oral small-molecule inhibitor of lysine gingipains—protease virulence factors secreted by the bacterium Porphyromonas gingivalis. *P. gingivalis* is best known for causing gum disease and has also been linked to the development of Alzheimer’s disease. COR588 is designed to inhibit the activity of these toxic proteins (gingipains), which are found in the brains of patients with Alzheimer’s and are thought to trigger inflammation around nerve cells, leading to neurodegeneration and cognitive symptoms. The drug was developed as an improvement over Cortexyme's earlier candidate atuzaginstat (COR388), offering better pharmacokinetic properties suitable for once-daily oral dosing and efficient central nervous system penetration. COR588 has completed Phase 1 clinical trials in healthy adults, demonstrating safety, tolerability, dose-proportional pharmacokinetics with an 11–12 hour half-life, and no serious adverse events.
