Cord blood platelet-rich plasma (CB-PRP) is an investigational biologic therapy being developed by Fondazione IRCCS Ca' Granda, Ospedale Maggiore Policlinico for the treatment of vulvar lichen sclerosus (VLS). Unlike autologous PRP, which is derived from the patient's own peripheral blood, CB-PRP is prepared from allogeneic umbilical cord blood, which is hypothesized to contain a more potent profile of regenerative factors. The therapy involves the local injection of concentrated platelets into the affected vulvar tissues. Upon activation, these platelets release a variety of growth factors and cytokines, including PDGF, TGF-β, and VEGF, which promote tissue repair, stimulate angiogenesis, and modulate the local immune response. This regenerative approach aims to alleviate the chronic inflammation and scarring characteristic of VLS, particularly in patients who have not achieved adequate relief with standard topical corticosteroid therapy.
