This is a combination therapy consisting of three drugs: - **Corticosteroids** are synthetic analogs of endogenous glucocorticoids, acting as potent anti-inflammatory and immunosuppressive agents. They modulate gene expression to suppress pro-inflammatory cytokines and immune cell activation. - **Colchicine** is an alkaloid that disrupts microtubule polymerization, inhibiting neutrophil motility and activity, thereby exerting anti-inflammatory effects. - **Pirfenidone** is an oral antifibrotic agent with anti-inflammatory and antioxidant properties. It inhibits the synthesis of profibrotic mediators such as transforming growth factor-beta (TGF-β), reducing fibroblast proliferation and collagen synthesis. The combination has been explored in experimental settings for conditions involving inflammation and fibrosis, such as idiopathic pulmonary fibrosis (IPF), post-COVID pulmonary fibrosis, connective tissue disease-associated interstitial lung disease (CTD-ILD), and other fibrosing or inflammatory disorders. The rationale for combining these agents is to leverage their complementary mechanisms—corticosteroids for rapid immunosuppression, colchicine for targeted inhibition of neutrophil-driven inflammation, and pirfenidone for direct antifibrotic action[2][3][4][5].
