Overview

This is a combination therapy consisting of three distinct agents: - **Corticosteroids** (such as prednisone, methylprednisolone, or dexamethasone): synthetic analogs of endogenous glucocorticoids with potent anti-inflammatory and immunosuppressive effects. They act by binding to the glucocorticoid receptor and modulating gene expression to suppress pro-inflammatory cytokines and immune cell activation. - **Intravenous Immunoglobulin (IVIG)**: a blood product containing pooled IgG antibodies from healthy donors. It exerts immunomodulatory effects through multiple mechanisms including Fc receptor blockade on phagocytes, neutralization of autoantibodies, modulation of complement activation, and regulation of cytokine production. - **Anti-D immunoglobulin** (Rho(D) immune globulin): a human-derived antibody targeting the RhD antigen on red blood cells. In immune thrombocytopenia (ITP), it works by opsonizing RhD-positive erythrocytes for clearance in the spleen, thereby diverting macrophage activity away from platelet destruction. This combination is used primarily in severe or refractory cases of autoimmune cytopenias such as immune thrombocytopenia (ITP), multisystem inflammatory syndrome in children (MIS-C), Kawasaki disease resistant to standard therapy, and occasionally toxic epidermal necrolysis. The rationale for combining these agents is to leverage their complementary mechanisms—rapid increase in platelet count via IVIG/anti-D and sustained suppression of autoimmunity/inflammation via corticosteroids[1][2][5][7][8].
