This investigational combination therapy consists of a corticosteroid (typically prednisolone) and the anti-fibrotic agent pirfenidone. It is being evaluated by Mansoura University for the treatment of post-COVID-19 pulmonary fibrosis. Corticosteroids provide broad anti-inflammatory effects by modulating gene expression through the glucocorticoid receptor, while pirfenidone is a small molecule that inhibits the synthesis of transforming growth factor-beta (TGF-beta), a key mediator of fibrogenesis, and reduces fibroblast proliferation and collagen deposition. The combination aims to address both the inflammatory and fibrotic components of lung injury following severe COVID-19 pneumonia.
