Overview

Cosagliptin is a novel, orally administered small molecule belonging to the class of dipeptidyl peptidase-4 (DPP-4) inhibitors. Developed by Haisco Pharmaceutical, its primary indication is for the improvement of glycemic control in adults with type 2 diabetes mellitus. The drug functions by inhibiting the DPP-4 enzyme, which in turn increases the circulating levels of active incretin hormones, such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). This leads to enhanced glucose-dependent insulin secretion and suppressed glucagon release, ultimately resulting in reduced blood glucose levels. A notable feature of cosagliptin is its extended pharmacokinetic profile, allowing for a convenient bi-weekly (every 14 days) dosing regimen, which is expected to improve patient adherence. It has also been noted that cosagliptin does not require dose adjustment for patients with renal impairment.
