Drug intelligence / Profile preview
COT-832 is a novel polyene macrolide antifungal being developed by Shionogi. Created through the structural modification of amphotericin B, COT-832 is designed to maintain the broad-spectrum fungicidal efficacy of parent polyenes while significantly reducing associated toxicities, such as hemolysis and cytotoxicity. In preclinical studies, COT-832 demonstrated potent in vitro activity against clinical isolates of Candida, Aspergillus, Mucor, and Cryptococcus, including azole-resistant and echinocandin-resistant strains. In rodent models of systemic candidiasis and invasive pulmonary aspergillosis, intravenous administration of COT-832 showed superior fungicidal efficacy compared to liposomal amphotericin B (AmBisome) at clinically equivalent doses, while maintaining a comparable safety profile.
