Overview
CPI-613 (devimistat) + mFOLFIRINOX is a combination therapy under investigation for the treatment of metastatic and locally advanced pancreatic cancer. CPI-613 is a novel lipoate analog that targets mitochondrial metabolism by inhibiting two key enzymes of the tricarboxylic acid (TCA) cycle—pyruvate dehydrogenase and α-ketoglutarate dehydrogenase—thereby disrupting energy production in cancer cells preferentially[1][3][5]. Modified FOLFIRINOX (mFOLFIRINOX) is a chemotherapy regimen consisting of reduced doses of oxaliplatin, irinotecan, leucovorin, and 5-fluorouracil to improve tolerability while maintaining efficacy[2][4][5]. The combination aims to enhance anti-tumor activity by sensitizing tumor cells to chemotherapy through metabolic disruption. Clinical trials have shown promising response rates with manageable toxicity profiles in patients with metastatic pancreatic cancer[1][2][3].
